This is a mandatory field and if the CMN is sent, this field must be filled in or the claim will be denied. GD0 field 37, positions 126 - 140, is the ordering provider ID number. This is the ID number of the doctor who prescribed the durable medical equipment for the patient. This field is also mandatory and must be filled in if this CMN is transmitted. GD0 field 38, positions 141 - 150, is the ordering provider phone number field. This field transmits the phone number of the doctor who prescribed the durable medical equipment for the patient. GD0 field 40, position 159, is the certification on file indicator. All CMNs, even though they can be transmitted electronically, have to have a paper file copy. This is required by law. So it may seem strange that this field would even exist. The reason it does is that the claim can be electronically transmitted before the actual CMN is on file. This allows for speedy claims processing. However, if audited and no CMN is found to be on file for this patient, serious repercussions will be felt by all parties involved.
